Via the Little Kanawha and Ohio Rivers, it is part of the watershed of the Mississippi River, draining an area of in a rural region on the unglaciated portion of the Allegheny Plateau. The Sand Fork rises approximately northwest of Roanoke in Lewis County and flows west-southwestward into eastern Gilmer County, where it flows into the Little Kanawha River from the north in the town of Sand Fork. According to the West Virginia Department of Environmental Protection, approximately 92.8% of the Sand Fork watershed is forested, mostly deciduous.
